Mark your calendars for the October 2007 New Orleans mission trip, to be held October 13 - 27. 

Here are the items you need to attend the trip:

1. A tetanus shot within the last 9.5 years (if you're not sure, get another one)
2. Pay the trip participant fee.  Click the button to pay online: 

Three documents need to be completed before you can participate in this trip. In addition, you need to have had a Tetanus shot within the past 9.5 years. We do want everyone to complete a skills survey, regardless of whether you have been on a previous trip.  Also, because all the previously completed health related documentation was shredded at the end each trip to protect your privacy. you will have to provide that information again along with the release documents for the October 2007 trip. 

1. Print the  “General Release Form"
   
   1. Fill it out completely (2 pages)
   2. Sign it




2. Print the “Health Form” 
   
   1. Fill it out completely (2 pages)
   2. Sign it
   3. Copy the front and back of your insurance card and staple it to this form
   4. Seal the form with your insurance card in an envelope with your name and date on it
3. Print the “Skills Survey”
   
   1. Fill it out completely (1 page)

Put all completed documents in Denny McGurer's church box, or mail it to Denny at 1040 Orchard Pond Court, Lake Zurich, IL 60047.
